This proposal covers a WordPress brochure website, initial business setup, and an optional CRM build for Phenomenal Health Care LLC. The website will serve as the front door for new client requests and job applications. The CRM, built on GoHighLevel, will manage intake, communication, and follow-up in a HIPAA-compliant environment.

Minnesota Web Studio is led by Matt Sommers, PharmD, bringing a clinical healthcare background to every project involving patient-facing or care-related digital systems.

New client request and job application forms built and embedded from the website
Automated follow-up sequences for prospective clients and job applicants

A Note on Caretap EVV Integration — We have reached out to Caretap's team to determine whether their platform offers an API, webhooks, or another technical path to connect with GoHighLevel. As of this proposal, we have not yet received a response. Because of this, the scope above draws a clear line between two types of CRM automation:

Prospect and applicant communication: fully deliverable regardless of Caretap's answer, since this data originates in GoHighLevel itself.
Automated follow-up for existing clients tied to visit or schedule data: dependent on what Caretap's platform exposes. If a viable connection exists, this will be scoped and priced as a follow-on phase once we hear back.

This approach lets us move forward on a firm timeline today without committing to integration work whose feasibility isn't yet confirmed.
